2. (If you mailed your OPT/STEM OPT packet out to USCIS): Where did you send it to? I-797C says National Benefits Center, U.S. Citizenship & Immigration SVC, P.O. Box 25920, Overland Park KS 66225
3. (If you filed the online OPT/STEM OPT application) The date you submitted the online OPT application: N/A
4. (If your OPT/STEM OPT has been approved) The date you were notified that your OPT/STEM is approved. It can be found in the I-797C: 12/1/2021
5. (If you have received your EAD card) The date you received the EAD card: 12/3/2021
