OPT approved but wrong OPT start date on EAD and SEVIS

Please follow the writing format:

1. (If you mailed your OPT/STEM OPT packet out to USCIS) The date that you sent out the OPT application packet to USCIS: 10/04/2021

2. (If you mailed your OPT/STEM OPT packet out to USCIS): Where did you send it to? Chicago Lockbox

3. (If you filed the online OPT/STEM OPT application) The date you submitted the online OPT application: N/A

4. (If your OPT/STEM OPT has been approved) The date you were notified that your OPT/STEM is approved. It can be found in the I-797C:
10/29/2021- However the EAD card had the wrong OPT start date so does the SEVIS record. Due to the error, more than 200 days of unemployment were accrued. To fix the issue, two actions were taken; 1. completed the USCIS typographic error form 2. assembled the EAD card replacement packet including

• The letter: refer to the blog post on SEVIS SAVVY
• The Form I-765 that the student filed initially to apply for OPT
• The I-20
• The letter from DSO
• Current EAD card with wrong OPT start date
• A copy of the biographical page of your passport

These were sent to USCIS National Benefits Center Attn: I-765 P.O. Box 648004 Lee’s Summit MO 64064.

5. (If you have received your EAD card) The date you received the EAD card: 11/18/2021, the correct EAD was received on 2/7/2021
