KeymasterPlease follow the writing format:
1. (If you mailed your OPT/STEM OPT packet out to USCIS) The date that you sent out the OPT application packet to USCIS: 09/13/2021
2. (If you mailed your OPT/STEM OPT packet out to USCIS): Where did you send it to? Chicago Lockbox
3. (If you filed the online OPT/STEM OPT application) The date you submitted the online OPT application: N/A
4. (If your OPT/STEM OPT has been approved) The date you were notified that your OPT/STEM is approved. It can be found in the I-797C:
N/A- denied due to no recommendation in SEVIS. The student didn’t meet with the DSO prior to the OPT filing; this resulted in no OPT recommendation in SEVIS and the student didn’t include the OPT recommended I-20. After receiving the denial notice, the student reached out to a DSO and the DSO made an OPT recommendation on the student’s SEVIS record on 10/18/2021. DSO helped the student to write a letter explaining that it was the student’s mistake/error not to go through the SEVIS recommendation process. The OPT was approved on 11/16/2021. (The student paid the filing fee twice.)5. (If you have received your EAD card) The date you received the EAD card: 11/18/2021
